After struggling a bit with the table of contents procedure, I've come up with a slightly different method that works really well for me.
1-type in a "table of contents" header at the top of your article
2-make a bookmark of the area you want to add to the table of contents
3-place the cursor beneath the "table of contents" header
4-make a hyperlink of the bookmark
just repeat the procedure as desired
